Axess Pointe Community Health Center
Axess Pointe Community Health Center, formerly called Akron Community Health Resources, is a not-for-profit health center that delivers primary medical, dental, mental health, and preventive health services. Through a Medication Access Program (described below), you can also find help with medications.
With locations in Akron, Barberton and Kent, Axess Pointe provides diagnosis, treatment, and preventive health services to residents of Summit County and accepts Medicaid, Medicare and private insurance. The agency sees a substantial number of uninsured patients and offers a sliding scale fee program for uninsured and under-insured patients who apply for medical and dental services. Fees are based on annual income and household size.
Patients are responsible for payments as determined by a formula based on annual Federal Poverty Level standards. Fees start at $25 for
a medical visit and $50 for a basic dental visit, or are determined as a percentage of cost based on income, whichever
is greater. Proof of income is required to determine eligibility for this program. Payment is expected before
services are provided at each visit.

You should call ahead for an appointment at 330.724.5471, but walk-ins are welcomes. You should bring insurance cards and/or proof-of-income information, such as pay stubs or a federal tax return.
Medication Access Program
Axess Pointe has multiple programs to help patients with their medications through a program in cooperation with Northeastern Ohio Universities College of Pharmacy.
An Axess Pointe pharmacist will help you find an appropriate program to make your medications more affordable.
In addition, a specially trained pharmacist will perform a comprehensive medication review during your visit to ensure that your medications are appropriate, effective, safe and convenient for you. The pharmacist will provide you with information about your medications and disease states to help you get the most benefit from your medications.
